DESCRIPTION:
A family guide to transition planning with your student and IEP Team to prepare your child for life after high school:
- Assess their child’s strengths, interests, and needs for successful transition planning
- Establish measurable post-secondary goals and annual goals
- Identify appropriate transition services and coordinated activities
All NYC families are welcome to attend.
