Care Coordination Plus (CCO+)
Better Mental Health Support for New Yorkers with IDD
Many people with intellectual and developmental disabilities (IDD) also have mental health needs—but services don’t always work together. CCO+ strengthens care so people get timely support without unnecessary ER visits or hospital stays.
A situação atual
- New York’s CCOs support about 130,000 people with IDD, many with co‑occurring mental health needs who struggle to get timely care.
- Because systems aren’t well coordinated, people face delays in psychiatric care, over‑reliance on emergency departments, and unnecessary hospitalizations.
Our Proposal – Establish a CCO+ Pilot Program
- Establish a CCO+ pilot (via an amendment to the Mental Hygiene Law) so qualified CCOs can provide:
- Intensive, multidisciplinary team‑based care management.
- 24/7 behavioral health access.
- Enhanced Medicaid reimbursement to fund proactive mental health support.
- CCO+ strengthens existing CCO services and fills critical gaps for people with both IDD and mental health needs.
