What Is This Project?

The New York State Office of the Chief Disability Officer (OCDO) and Positive Exposure are working together on a special book project.

The book will celebrate the beauty, diversity, and life experiences of New Yorkers with disabilities. It will include professional photographs taken by photographer Rick Guidotti, along with personal stories, quotes, and biographies.

Who Can Be Nominated?

People with disabilities who live in New York State can be nominated for this project.

You can:

  • Nominate yourself
  • Nominate someone else (with their knowledge and permission)

Important Information About Disability Disclosure

This project is about sharing and celebrating the experiences of people with disabilities.

To participate, individuals must agree to share information about their disability with the OCDO and Positive Exposure.

Does a Nomination Guarantee Selection?

No.

Submitting a nomination lets the project team know you are interested in being considered. Not everyone who is nominated will be selected for the book.

If Someone Is Selected

People who are chosen for the project will be asked to sign consent forms before their photographs, stories, quotes, or disability information are used.

Privacy and Contact Information

The nomination form requires the full name, email address, and phone number of both the nominator and nominee.

This information will be kept confidential.

Nomination Deadline

Nominations will be accepted through August 14, 2026.
