Link to Better Health can help people without Care Management

Link to Better Health is a program funded by a five-year grant from the Centers for Disease Control (CDC) that can help adults with IDD improve their health through education and support in accessing needed healthcare services. This free, short-term service is for New York residents not enrolled in Care Management and is subcontracted by TriaDD, LLC. TriaDD is a statewide partnership between three CCOs created to help improve care for New Yorkers with IDD. ACANY and LIFEPlan CCO are TriaDD members, along with Person Centered Services in Western New York. 

Benefits:

  • Help finding medical providers and coordinating medical appointments
  • Assistance getting Medicaid/insurance coverage for medical expenses
  • Connections with social services, such as SNAP, HEAP, and more
  • Help navigating the health care system
  • Connections to preventative services, such as screenings and well-care visits
  • After completion of pre and post knowledge assessments, participants may receive up to $100 in VISA Gift Cards, pending availability in their region

Eligibility Criteria

  • New York State resident
  • Age 18+ 
  • Self-identify or have a diagnosis of IDD; OPWDD eligibility is not required 
  • Not enrolled in a Care Management program 

Gift cards are available to qualifying participants who complete pre- and post-knowledge assessments as part of the free, 90-day service. Please visit TriaDD, LLC, to refer adults with unmet healthcare needs who want to improve their health. Enrollment in Medicaid or another health insurance plan is not required.
