Support for adults with IDD who are not yet enrolled in Medicaid or Care Management
Link to Better Health is a free, short-term program designed to help adults with intellectual and developmental disabilities (IDD) improve their health and access needed services.
Funded through a five-year grant from the Centers for Disease Control (CDC), this program provides personalized guidance, education, and support—especially for individuals who are not yet connected to Medicaid or a Care Management program (CCO).
The program is administered by TriaDD, LLC, a statewide partnership of three Care Coordination Organizations (CCOs):
- ACANY
- LIFEPlan CCO
- Person Centered Services
Who This Program Is For
Link to Better Health is intended for individuals who:
- Live in New York State
- Are age 18 or older
- Identify as having, or have a diagnosis of, an intellectual or developmental disability (IDD)
- Are not currently enrolled in Care Management or a CCO
- May not yet have Medicaid or other health insurance
OPWDD eligibility is not required.
How We Can help
Participants receive one-on-one support to:
- Find healthcare providers and schedule medical appointments
- Apply for Medicaid or obtain other health insurance coverage
- Navigate the healthcare system with confidence
- Connect to community resources such as SNAP, HEAP, and other supports
- Access preventive care, including screenings and wellness visits

Link to Better Health is designed to help individuals take the first steps toward better health—especially those who may feel overwhelmed or unsure about how to access care. The program acts as a bridge to services, helping people get connected before enrolling in long-term supports like Care Management.
