The CMOR

Care Management Observation Report 

The Care Management Observation Report or “CMOR” must be completed by Care Management to document the health and safety of Willowbrook Class Members residing in certified settings. During the Public Health Emergency (PHE), the requirements for CMORs were waived. Now that the PHE has ended, Care Managers are required to complete CMORs for each Willowbrook Class Member as they did before the pandemic.

Closer look at the requirements

Willowbrook Care Managers must complete the CMOR during a home visit at least two times in a calendar year. It is required that the CMOR is completed twice a year, in non-consecutive quarters (for example, the first and third quarters). The Member must always be present in the home during the completion of a CMOR.

Only one report needs to be completed when visiting more than one individual living in the same certified residence or Family Care home. Still, the report must reflect the input of each of the individuals.

As applicable, at least one CMOR per year must be completed with the local CAB representative.

If no problems are observed and reported

The CMOR will be reviewed with the Member and staff. A copy of the CMOR will be retained at the residence (except for a Member living in a Family Care home). For an individual living in a Family Care home, the Care Manager must send a copy of the CMOR to the Family Care liaison responsible for the home.

If problems are observed and reported

The CMOR will be reviewed with the Member and staff, and a copy of the CMOR will be retained at the residence (except for an individual living in a Family Care home). The Care Manager must send a copy of the CMOR to their supervisor. The Care Manager’s supervisor must send a copy to the executive director of the voluntary agency operating the residence or to the DDSO director for state-operated residences. For an individual living in a Family Care home, the Care Manager’s supervisor must also send a copy to the Family Care home’s liaison and to the Agency/DDSO Family Care Coordinator. Care Managers are expected to complete follow-ups until the issues are resolved.
