ACANY has a clinical team to enhance Care Managers’ support for members with complex medical or behavioral health needs. The team is made up of medical and behavioral health professionals who are experienced in working with people with IDD. They provide consultations to Care Managers and develop training and resources for Care Managers, members, and families. Please note that the Clinical team does not provide direct services to members or families. Their goals are:
- Identify members at risk of a future crisis
- Increase preventative strategies and monitoring to prevent crises and improve health outcomes
- Decrease unnecessary hospital utilization and preventable readmissions
In the coming weeks, the Directors of Provider Relations will begin to work with select Providers to help identify their most vulnerable members. The members’ information will be provided to our clinical team so they can ensure that those with the highest needs are not overlooked. This will be achieved through collaboration to develop creative strategies for acute concerns, recommendation of evidence-based supports and services, and quality reviews to ensure accurate and updated medical records.
